Coverage You Can Count On

We Accept Most Major Insurance Plans

We currently accept Medicare, Aetna, Cigna, UnitedHealthcare, Optum, and more. Coverage varies by individual plan. Please contact your insurance company to confirm that our provider is in network.
Important: Participation with an insurer (e.g., Aetna, Blue Shield, Cigna) does not necessarily include all plans or products offered by that insurer. Please verify your coverage with our office prior to scheduling.
Insurance Disclaimer: Insurance participation and benefit verification do not guarantee payment. Patients are responsible for confirming that their provider is in network and for paying any applicable deductibles, copayments, coinsurance, non-covered services, and denied claims. Coverage and benefits vary by plan, and patients are encouraged to verify their specific insurance benefits prior to receiving services.

Insurance Verification Process

Complete these simple steps before your first appointment so we can verify your insurance coverage and provide an estimated patient responsibility.

1. Submit Your Insurance Card

Provide a clear copy of the front and back of your current insurance card so we can begin the verification process.

2. Provide a Government-Issued ID

Submit a valid government-issued photo identification to confirm your identity and ensure accurate patient records.

3. Share Subscriber Information

If you are covered under another person's insurance, please provide the subscriber's full name, date of birth, and relationship to the patient.

4. Eligibility & Benefits Verification

Our team will verify your insurance eligibility and available benefits before your appointment to help identify your expected coverage.

5. Receive Your Cost Estimate

Before your first visit, we'll provide an estimated patient responsibility, including any anticipated copayments, coinsurance, deductibles, or other out-of-pocket costs based on the information available.

Secure Payment Options

We accept major insurance plans and multiple payment types to keep billing clear and stress free. Our team helps you understand coverage, payments, and next steps before your visit.

We accept major credit and debit cards, HSA/FSA cards, cash, and secure electronic payments. If accepted, checks may also be used for payment. A valid card may be required to remain on file, and payment plans may be available when applicable. Please contact our office with any questions about insurance participation, benefit verification, copayments, billing, or payment options.

Good Faith Estimate

Your Right to a Good Faith Estimate

Under federal law, patients who are uninsured or choose not to use their insurance have the right to receive a Good Faith Estimate explaining the expected cost of their medical care before services are provided.

If you schedule an appointment or request an estimate, we will provide you with a Good Faith Estimate of expected charges in accordance with applicable federal requirements. If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges.

Please contact our office if you have questions or would like to request a Good Faith Estimate before your appointment.

Out-of-Network Coverage

We do not participate with out-of-network insurance plans. We do not provide superbills, submit out-of-network claims, offer sliding-scale rates, or accept secondary insurance. Payment in full is required at the time of your appointment.

Please note: Patients are responsible for verifying their insurance benefits and for any costs not covered by their insurance plan.
